Hair transplant statistics: what the ISHRS census really shows, including the “95%” line it misreads
Hair transplant statistics, corrected: 703,183 procedures in 2021, 15.3% women in 2024, and why 95% is misread.

You need one number for a story, a clinic page, or a decision you have been postponing. The cleanest answer is 703,183 surgical hair restoration procedures worldwide in 2021—the latest worldwide estimate, not a 2024 count. ISHRS says the true worldwide number is unknown.
The newer 2025 Practice Census covers 2024 but is a voluntary physician-member survey. It informs technique, age, sex and practice patterns, not every provider or patient.
This page corrects three common claims: that 95% of first-time patients are aged 20–35, that female patients rose 16.5%, and that graft survival equals success. Graft survival is not success.

How many hair transplants are done each year?
The best available worldwide estimate is 703,183 surgical procedures in 2021. It shows scale, but is not a registry count: ISHRS built it from member averages and estimated market share, then said the true worldwide total is unknown.
The estimate was 4% below 2019’s 735,312 and 152% above the 2010 estimate of 279,381. COVID-19 affected elective surgery in 2021; the 2024 census published no new worldwide extrapolation.
In the 2021 estimate, Asia and Australia led with 256,835 procedures, followed by the United States and Canada with 149,254. The report does not break out Turkey.
Caveat: best available reference, not a precise annual total.
Are hair transplant patients getting younger and is the 95 percent claim true?
No. The widely repeated 95% line does not mean 95% of patients were aged 20–35; it describes physicians’ average-age answers. The press release paraphrased a physician-level average as a patient share.

The same census places 17.9% of male surgical patients under 30 and women at 18.3%. Neither figure fits the idea that almost every patient is 20–35.
Caveat: broad age bands cannot give an exact 20–35 patient share.

Are more women getting hair transplants?
Women represented 15.3% of surgical hair restoration procedures performed by ISHRS members in 2024. That is meaningful demand, not a takeover: the female share stayed between 12.7% and 15.8% across reported years.

The press release says female surgical patients increased 16.5% from 2021, but that figure cannot be reproduced from the published tables: the female share rose from 12.7% to 15.3%, a 20.5% relative rise, while per-member averages imply about 14%. Women’s share simply returned to its 2014 level.
Women were more likely than men to receive non-scalp procedures: 21% of women’s procedures versus 18% of men’s, with eyebrows leading for women at 12.2%.
Caveat: physicians’ estimated shares, not a verified patient count.
Is FUE now the standard technique and what happened to robots?
FUE is now the dominant harvesting method in the ISHRS data, especially for men. In 2024 it covered 85.4% of men’s procedures; strip harvesting, or FUT, accounted for 12.5%. Preference does not guarantee outcome.

The shift is not a robot boom: robotic-assisted FUE was 5.3% of FUE procedures in 2019 and 6.4% in 2021, then fell to 1.3% in 2024, while hand-held motorized FUE without suction reached 75.1% in 2024. The FDA’s ARTAS clearance covers computer-assisted harvesting for men with androgenetic alopecia and black or brown straight hair. A 2024 study of 13 men found robotic graft yield lower than manual FUE, 82.05% versus 90.03%, though that yield gap was not statistically significant.

What is the real hair transplant success rate?
There is no single hair transplant success rate that answers the patient’s real question. The useful reframe is graft survival is not success: a follicle can survive while density, hairline design, donor management or expectations disappoint.

Li’s study warns against marketing shorthand: 19% went back for another procedure despite high survival. A 2019 review found only three of 30 hair-loss studies used a patient-reported outcome measure. In the 2024 census, members reported 1.5 procedures on average; 67.3% said one was typical and 30.8% said two.
Clinic marketing pages we checked on 10 September 2026 advertise “95% to 98% graft survival” without citing a study. Even where survival is that high, it is not a satisfaction statistic. One page attributes a Turkey market-share figure to ISHRS, though none of the census editions we checked mentions Turkey.
Caveat: different follow-up periods and methods make survival percentages non-interchangeable.
How common are complications?
Most problems are temporary, but no honest single complication rate exists because studies define and count events differently. A 2025 scoping review found overall rates of 1.2% and 4.7% in two large series, while a meta-analysis counted complications in 442 of 2,353 patients, or about 18.8%, across heterogeneous observational studies.
The range is easier to understand by event:
- Folliculitis: 12.11% of 1,317 patients; multicenter.
- Shock loss: 23 of 621, including 14 of 67 women and 9 of 554 men; single-center FUE.
- Edema: 8.94% of 1,167 with one protocol versus 2.66% with modified prevention; 1.4% severe periorbital.
- Perifollicular erythema: 22.4% of 1,090; 0.92% severe.
- Sterile folliculitis: about 7.0% of 2,896 in a 10-year audit; zero major or life-threatening complications.
The scoping review reported persistent numbness and infection each up to 11%, hypertrophic scarring or keloid after FUT up to 15.1%, crusting up to 54.8%, frontal edema up to 50%, and sterile folliculitis up to 53.3%. Definitions change apparent risk.
Caveat: “up to” is a cross-study maximum, not every patient’s expected risk.
How big is the black market hair transplant problem?
The ISHRS data support a real safety concern, but not a country-by-country market ranking. In 2025, 59.4% of responding members said black-market clinics operated in their city, based on 153 responses; “black market” means clinics where unlicensed, non-physician staff perform surgical steps.
Members reported an average 10% of their 2024 cases as repairs of previous black-market procedures, with a median of 5%. That rose from a mean of 6% and median of 2% in 2021, though repair practices may see disproportionate failures.
Official health-tourism figures count all healthcare visitors—1,506,442 in 2024 and 1,398,580 in 2025—not hair-transplant patients. A 2025 review calls Turkey, especially Istanbul, an industry epicenter and cites about US$1 billion annually, but labels the estimate secondary and the sector poorly standardized. No saved ISHRS census edition reports Turkey’s global share.

What do these hair transplant statistics leave out?
They leave out the measurements a patient needs most: a verified 2024 worldwide count, medical-tourism share, per-graft price, and universal definition of success. The 2025 census is the best practice snapshot, but still voluntary.
The census received 247 responses from 946 ISHRS physician members with valid emails, a 26% response rate, and stated a margin of error of ±5.4% at 95% confidence. It was self-reported, region-weighted, and neither ISHRS nor its consultant independently verified responses.
