Refina AI Review: What the Plastic Surgery Preview App Costs and Can’t Tell You

Refina shows its price only after you upload your face, ships photos to about ten AI providers, and says its previews are not predictions. Read this first.

You saw the before-and-after clips on TikTok, opened the App Store, and found a polished promise: choose a procedure, upload your face, see a photoreal preview. Refina AI looks like it could make a consultation feel less abstract.

Then the important questions arrive late. What will it cost, what exactly is being generated, and where do those photos go before you see anything?

Refina is a real iOS app from Trilogy Labs LLC, but it is a paid AI image generator, not a surgical consultation. Its own policy says previews are not predictions and the app is not supervised by physicians; the price appears only after photo upload, and most written reviews complain about charges.

What is Refina AI and what does it do?

Refina edits your photos to visualize selected cosmetic procedures. Its listing names rhinoplasty, facelifts, chin augmentation, lip enhancement, jawline contouring, brow lift, blepharoplasty, neck lift, Botox and buccal fat removal, plus a 22-page AI dossier on proportions, symmetry, skin, color harmony and features.

You upload a front photo; side and tilted views unlock deeper analysis. There is also an AI Advisor, a dossier chat and short “Bring to Life” motion clips.

What it does not do matters more. It does not examine tissue, plan an operation or replace an in-person consultation. The store calls the dossier “clinic-quality,” while the privacy policy says its observations are AI-generated, approximate and may be inaccurate. Those are not equivalent promises.

The app may help turn a vague wish into a question for a doctor. That is a narrower use than treating its output as a treatment plan.

Is Refina AI legit?

Refina is a legitimate App Store product sold by Trilogy Labs LLC, a United States company that also lists Ink & Sky and FloraFinds. It is not the same business as refina.app, which redirects to an unrelated Shopify product advisor. Legit here means identifiable software and seller—not medical validity or a good billing experience.

The current version was released October 9, 2026. The policy names its former internal name, “EnhanceMe,” and gives team@trilogy-labs.com as the contact.

The written reviews are less comfortable. The full histogram is mostly five-star, but of the 35 written reviews, 26 are one star and 17 mention charges, cancellation or refunds. That sample is not the whole customer base, but it is too pointed to ignore.

There are positive experiences too. One five-star reviewer wrote that realistic previews “helped me ask better questions” before a consult. That is a plausible use case; it does not make the images medically dependable.

What does Refina AI cost?

Refina is free to download, but the previews and dossier are paid—and you see the offer only after onboarding and photo upload. The App Store lists eight weekly price points from $6.99 to $19.99 and a $102.99 yearly plan.

The $14.99 tier, the one written reviews mention most, becomes $779.48 over 52 weeks (arithmetic). Even the cheapest $6.99 tier is $363.48 a year. At $14.99 a week you pass the yearly price in about seven weeks.

Why can two people see different prices? Refina’s policy says AppsFlyer and Superwall help choose which subscription offer each user is shown. There is no single universal quote.

The placement is the bigger issue. A third-party flow teardown shows photo upload, account creation, dossier generation, and only then the paywall. Several reviewers describe learning the price after submitting photos.

Variable offers are not automatically improper. But read the exact renewal amount and period on Apple’s confirmation sheet before you approve it.

Is Refina AI accurate?

Refina does not claim its previews predict surgery. Its policy calls them illustrative renderings, not a simulation of surgery, and says the dossier is not a clinical assessment. The app is not operated or supervised by physicians—so “accurate” can only mean visually useful, not a surgical forecast.

That is the core problem with the category. A render is not a result: a generated preview shows what an image model can draw on your photo, not how tissue behaves, how healing unfolds, or what a surgeon will produce.

Users report reliability problems with the dossier too. A two-star reviewer wrote that it “told me I had narrow set eyes on one page, wide set eyes on another,” and that it counted shadows as blemishes—“about as accurate as a horoscope.” A one-star reviewer said a paid result stayed “almost ready” for most of five days.

Those are user reports, not controlled tests. But they show why a polished edit can create false confidence: realism and consistency are separate properties. What happens to your photos?

Your photos leave the device. Refina says facial photos, profile details such as age, gender and ethnicity, and your selected parameters are sent to third-party AI providers, and kept until you delete them or your account.

The named chain includes Replicate, OpenAI, Anthropic, OpenRouter and fal.ai. OpenRouter can route requests to Google’s Gemini, Alibaba Cloud’s Qwen and Moonshot AI’s Kimi; fal.ai routes motion clips to MiniMax. Processing may happen in the European Union through Firebase or in Singapore when Alibaba Cloud handles Qwen.

Refina says those providers’ API terms do not permit training on submitted data, but also that it cannot independently audit them. Images are “not sold” and not sent to advertising partners—but they are shared with processors. Deleted data is recoverable for seven days, encrypted backups are purged within 180 days, and purchase records are kept for ten years.

The App Store label also lists photos, sensitive information, contact details, identifiers and usage data as linked to you. Compare that with other apps in our face-rating privacy audit.

The provider list is unusually transparent. Still, “not used for training” is a contractual assurance, not an audit.

How do you cancel Refina AI and get a refund?

Refina sells through Apple in-app purchases, so cancel in Apple’s subscription settings: Settings → your name → Subscriptions → Refina → Cancel Subscription (Apple’s steps). If there is no Cancel button, it is already canceled.

For a refund, use reportaproblem.apple.com, choose “I’d like to,” then “Request a refund” (Apple’s refund guide). Apple says to allow 24 to 48 hours for an update.

The developer offers refund help at team@trilogy-labs.com, listed on its support page. Several reviewers say support never replied; one wrote: “No customer support! No response to email! No response to cancellation so the charges keep happening weekly!” Keep your Apple receipt and the cancellation screen.

Is there a free alternative to Refina AI?

Not for what Refina claims to do—and that is the point. No free tool, and no paid one, can show how your tissue will heal. If you are seriously considering a procedure, the right next step is an in-person consultation with a board-certified surgeon, not another render.

Refina AI is a real, polished product with one plausible use: preparing questions for a consultation. But the late, variable paywall, a sharply negative written-review sample and a long chain of AI processors make it a purchase that deserves care. Its own documents state the central limit: previews are not predictions, and the dossier is not clinical. A render is not a result.
