Testosterone statistics: the real numbers, and where the famous ones came from
Morning testosterone varies 18.7% between days. See the reference range, age trends, generation claim, and therapy evidence.

You have just read that men your age have a fraction of the testosterone your grandfather had, and you are deciding whether to pay for a blood test. The direct answer is that one result can be useful, but it is not a permanent label: testosterone moves with the hour, the day, illness, the assay, and the person.
The famous population claims are not all wrong. They are usually stripped of provenance. Before you treat a number as a verdict, ask when it was drawn, how it was measured, and whether the original paper said what the headline says it said.

What is a normal testosterone level?
There is no single normal number. In healthy nonobese European and American men aged 19 to 39, harmonized total testosterone ran from 264 to 916 ng/dL, median 531, drawn from 9054 community-dwelling men across four cohorts. Travison et al.
That is a reference population, not a promise about how you should feel. The AUA uses 300 ng/dL as its cut-off, but its own guideline says the threshold came from mean levels in the available literature, not from an outcome study. Diagnosis needs symptoms plus consistently low results — not one decimal.
How much does one man's testosterone move on its own?
Here is the number most testosterone arguments skip. Morning testosterone has 18.7% between-day biological variation, so it takes a 52% change between two morning draws before you can be confident anything real moved. Collier et al.
The clock adds another trap. In men aged 30–40 the 1600 h reading was 20–25% lower than at 0800 h; by 70 the gap narrows to 10%. Seventeen men who looked low in the afternoon were normal at all three morning visits — the diagnosis was made by the clock, not the man. Brambilla et al.
Call it the single-draw fallacy: reading one moving snapshot as a verdict. Acute respiratory infection alone lowered mean testosterone by 10% in young men, up to 30% in some cohorts. AUA guideline
Repeat testing is not caution for its own sake. The measurement has its own operating range, and that range is wide.
Is testosterone really falling across generations?
One influential paper did report an age-independent population-level decline in American men. Its abstract never states the famous “1% per year”. The Massachusetts Male Aging Study ran three waves — 1987–1999, 1995–1997, 2002–2004 — covering 2769 observations on 1532 men from greater Boston, all aged 45–79 at entry. Travison et al.
That design measured a population trend, not a comparison between a man and his grandfather. Later evidence is mixed. NHANES found essentially no change between 1988–1991 and 1999–2004; Danish surveys lost their apparent decline once rising BMI was accounted for; but a US Air Force veteran cohort still fell by 117 ng/dl over 20 years among men who held or lost weight, so obesity is not the whole story either. The “1% a year” wording is a later paraphrase, not a law anyone measured. The trend may be real; its cause and its rate are not settled.
How much does testosterone drop with age?
It declines, but the rate is an average, not a schedule. In the Baltimore Longitudinal Study of Aging 890 men averaged a within-man change of -0.124 nmol/L per year, and hypogonadal levels reached about 20% of men over 60, 30% over 70 and 50% over 80. Harman et al.
Age is one layer of several. Obese men were almost five times more likely to have low testosterone, and timing and illness move the result before aging explains anything. AUA guideline

What does testosterone actually do to a man's face and body?
Muscle and hair respond. The face is a different question. In the classic trial, testosterone plus exercise produced 6.1 ± 0.6 kg of fat-free mass in 43 normal men over 10 weeks, and testosterone alone raised triceps area without any training. Bhasin et al.
Then the split. In a 20-year follow-up, umbilical-cord testosterone predicted adult facial masculinity (r = -0.55) while adult circulating testosterone did not (r = 0.01, p = 0.93). Facial width-to-height ratio — the internet's “wide jaw” number — did not track adolescent testosterone either: one of five predictions held. Whitehouse et al. Hodges-Simeon et al.
Hair exposes the same shortcut. Men with 5-alpha-reductase deficiency carry higher plasma testosterone yet have a scant to absent beard and no temporal hairline recession — those are DHT traits. What about testosterone therapy?
Testosterone therapy treats confirmed hypogonadism. It is not a generic upgrade. The Endocrine Society requires symptoms or signs plus consistently low serum testosterone confirmed on a repeat morning fasting draw; the AUA requires two separate early-morning measurements. The safety story is more precise than either the sales copy or the panic. TRAVERSE randomised 5246 men with two fasting results below 300 ng/dL: a primary cardiovascular event hit 7.0% on testosterone against 7.3% on placebo. In 2025 the FDA dropped the cardiovascular boxed warning, kept the limitation for age-related hypogonadism, and added a class-wide blood-pressure warning. TRAVERSE FDA
Prescribing outran the evidence: it rose in 37 of 41 countries surveyed between 2000 and 2011 with no new approved indication, and among US men who started, only 74.72% had had testosterone measured in the previous year. What we could not verify
We searched the original Massachusetts Male Aging Study paper, the citation trail behind the “1%” figure, the NHANES trend analyses, the Danish surveys and the major guidelines. We could verify the original study’s population-level conclusion. We could not find a primary source stating that testosterone falls 1% every year as a universal rule — what we found was a later paraphrase. We also found no accepted single cause for the population trend, and no blood-test formula that predicts how a man’s face reads.
