It is 11:40 at night and you are in the bathroom holding your phone up at an angle you have never seen your own head from. The flash is on. Your hair is wet and pushed back. Put the phone down for a second. Almost everything about the way you just measured yourself was rigged to give you a worse number, and the chart you were measuring against was never designed to do what you were using it for.
What the Norwood scale actually is
The Norwood scale is a seven-stage visual classification of male pattern hair loss — a shared vocabulary so a dermatologist in one city and a surgeon in another mean the same thing by "significant frontal recession with early vertex involvement." It carries two names because the pattern was first classified by Hamilton in the 1950s and revised into the modern seven-type version by Norwood in the 1970s.
It describes where hair is thinning — not how fast, not why, not what happens next. A map, not a prognosis, and nothing in it predicts your endpoint.
Key numbers
- 7 stages, plus a separate Type A variant series for a different progression pattern. That is the entire scale.
- 50 to 100 hairs a day is the range routinely cited as normal adult shedding. Hair in the shower drain is not evidence of anything by itself.
- 6 to 12 months. The standard clinical window before judging whether a hair-loss treatment is working — roughly 1 cm of scalp hair growth per month is why. Anyone promising a verdict in six weeks is selling.

Caveat, and a real one: the scale is bad at diffuse thinning. Plenty of men lose density evenly across the top without the hairline moving at all, and the scale has nowhere to put them. If your parting is widening but your hairline is intact, the scale is not your tool — a doctor is.
The one-stage overcall
Stated as an argument rather than a statistic: most men self-assess one full Norwood stage worse than a clinician would. Not from catastrophising — from three mechanical errors in how the assessment gets done.
- Wet hair, pushed back. Wet hair clumps and reveals scalp that dry hair covers, and pushing it back exposes temples a natural fall conceals. Every chart illustration is dry hair, neutral position.
- Overhead light, downward camera angle. Shot from above under a bathroom downlight, every scalp shows through — photograph a full head of hair that way and it looks thinning too.
- The wrong baseline. You are comparing yourself to your hairline at seventeen, which was never permanent for anyone. The honest comparison is a photo from six months ago.
Fix all three and the number usually moves back — the difference between an accurate reading and an anxious one, and only the accurate one helps you decide anything.
What the scale is genuinely for: timing, and how to check it
The scale earns its keep in one way — it tells you when, not how bad.
The mechanism: susceptible follicles do not die suddenly. They miniaturise over successive cycles, each new hair finer, shorter and less pigmented, until a terminal hair is a barely visible vellus one. Follicles at the sides and back resist this — hence the horseshoe, and hence transplants from there continuing to grow where they are moved. The consequence is large: holding a miniaturising follicle is far easier than reviving a gone one. So the useful question is never "what stage am I?" but "am I moving?" — and that comes from photographs, not charts. Four steps to photograph it properly:
- Dry hair, natural fall. Not wet, not pushed back, not styled.
- Three fixed angles. Front, 45 degrees, crown from above. The same three every time.
- Same room, same light, same time of day. Indirect daylight, never overhead spotlights.
- Every three months, on a calendar reminder. Compare the photos, not your memory — memory is the least reliable instrument here.
Different at three months is worth taking to a doctor. Identical is worth relaxing about. Either way you know something, which is more than the mirror was giving you.
